Forms are due to Preucil School by Jan. 15 to allow time to get them signed and back to MCI by the hard deadline of Feb. 1.

Travel Insurance Form

Deadline March 15
All participants are required to return a form and either take the insurance or waive it.
Optional insurance that covers "Cancel for Any Reason" is available until April 1 for $244, but we have set the deadline as March 15, so we can get the forms to MCI by April 1.
Parents sign for minors.

Due with Insurance Form for all Students who purchase Travel Insurance.
For students who purchase insurance, benefits will be assigned proportionally between Preucil School and the student because the grant is covering $3,000 of the student package cost (or $2,000 for partial grant).
Expenses incurred for medical care or travel expenses beyone the package cost will not be subject to the assignment of benefits.
